Auckland Referral Eligibility
Referral to the Whanau Ora Service can be from General Practice, other health professionals, community groups whanau or self.
ALL AUCKLAND REFERRALS MUST MEET THE FOLLOWING CRITERIA:
– Reside within ADHB region
– 18 years and over (less than can be referred to child/youth Toi Tu)
– At risk or current Cardiovascular disease /Diabetes (ALL Maori and Pacific meet CV risk by target ethnicity. Other individuals according to CV risk screening or disease.)
– May have a long term condition eg COPD, Asthma, CVD, Diabetes
– Client has ability to improve and become self managing of health and well being from Nursing and/or Lifestyle Coach intervention.
– Discussed with and consent obtained from the individual client for referral to this service
EXCLUSIONS
– This service does not duplicate services already contracted for by the MOH, ACC or DHB for inpatient or community services (e.g. Patients receiving palliative care)
– Mental health/cognitive conditions and addictions are excluded. Stable Depression can be referred.
Referral forms can be obtained from the website and emailed/faxed or set up within Medtech and sent via Healthlink.

Counties Manukau Referral Eligibility:
Referral to the Whanau Ora Service can be from General Practice, other health professionals, community groups whanau or self.
All Adult referrals must meet the following criteria:
– Reside within Auckland DHB or Counties Manukau DHB region
– 16 years and over
– At risk or current Cardiovascular disease /Diabetes (ALL Maaori and Pacific meet CV risk by target ethnicity. Other individuals according to CV risk screening or disease.)
– May have a long term condition, eg. COPD, Asthma, CVD, Diabetes
– Client has ability to improve and become self managing of health and well being from Nursing and/or Lifestyle Coach intervention.
– Discussed with and consent obtained from the individual client for referral to this service
